Supervisor of Case Management:

LSSNY seeks qualified individuals to provide case management services to individuals and families affected by Hurricane Sandy.  The lead case manager’s major responsibilities will be to conduct in-depth intake and assessment of individuals and families who have been negatively impacted by Hurricane Sandy, and to assist those persons with accessing assistance and relief.  The lead case manager will be responsible for conducting an individual needs assessment for each client, and for referring clients to external programs and agencies for resources and services, such as medical, mental health, employment assistance, and counseling.  The lead case manager will also collaborate with each client to develop a long-term recovery plan.  Additionally, the lead case manager will supervise and train other case managers in providing these services.  Bachelor’s degree in social work or related field is required; MSW, supervisory experience, and experience in disaster response preferred.
