To enroll bring the following documentation with you:

Proof of your child’s birth (birth certificate, baptism record, alien registration card)
Two proofs of residence (utility bill, rental lease, etc.)
Proof of income (pay stub, W2 form, etc.)
Public assistance/Medicaid card (if applicable)
